FORMS OF PAYMENTS ACCEPTED

We accept Cash and Checks. Please note there is a $30.00 charge for all non-sufficient funds checks issued to Life Clarity Group for payment of services rendered. If a client presents more than one NSF check to Life Clarity Group, payment in the form of check will no longer be accepted.  Sliding Fee Scale and Reduced Rates Reduced fees are available in certain circumstances. If you have questions about this policy, please ask before your appointment. You will be responsible for verification and approval for reduced fees must be agreed in writing.  


Fees are due at the time of service. 

INSURANCE

Currently, Life Clarity Group accepts various insurance and EAP networks.   


Please contact your insurance provider to determine if you have mental health benefits and what you will need to submit for financial reimbursement of fees paid to a non-paneled mental health provider (this means a mental health professional who does not direct bill your insurance carrier). 


You may want to ask your insurance provider the following questions when determining what, if any, reimbursement you may expect to obtain from your insurance carrier:  

  • Do I have mental health insurance benefits? 
  • What is my deductible and has it been met?  
  • How many sessions per year does my insurance cover? 
  • What is the coverage amount per therapy session?  
  • Is approval required from my primary care physician?  

CANCELLATION POLICY

If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a $40 fee.
